My psoriasis arthritis is spreading should I be asking my doctor to attack it more?

I am a 29 year old woman who has psoriasis arthritis, I am on methotrexate injections every week but over the last 18 months the arthritis is spreading to my hands, ankles , toes and hips but I have seen 4 different locum specialists at the hospital and each one just says come back in 3 months, I have had my last 2 appointments cancelled by the hospital and I want to try and slow down the spreading of this, should I be pressuring them to do something?
