Bleeding but very little pain

It's been about five days now with my hemroids flaring out. Last night i applied preperation H the cream where you insert it through your anal afterwards there was just the usually uncomfortable feeling but this time there was a lot of blood no pain really and they have been bleeding for 4 days. I been using witch hazel as well they seem to start to go away but then come back. I have been thinking about going to the doctor today but i was just wondering if i would have to have surgery because i have been reading on the site and it seems very painful.
